If you receive Medicare or Medicaid benefits, we can help you understand the benefits available to you.  If you have questions regarding insurance coverage, or receive a questionable medical bill, we can help you find an answer.  We can assist you in determining whether you qualify for government benefits through other agencies.  We provide general information regarding Powers of Attorney, Guardianship, Medical Directives, Nursing Home Medicaid, and Do-Not-Resuscitate Orders.

F A M I L Y   C A R E G I V E R 
S U P P O R T   S E R V I C E S

We provide information on support/counseling groups, respite services and other agencies and services that aid family caregivers.  We can provide valuable information to the caregiver about the effects of stress, ideas to help you care for your loved one at home, and end-of-life issues.  We are also able to provide relief services for caregivers.

M E A L S   A N D   
T R A N S P O R T A T I O N

The Area Agency on Aging contracts for congregate (group) or home-delivered meals.  We provide transportation to congregate meal sites as well.

C A R E    C O O R D I N A T I O N

Maintaining your independence while temporarily confined to home due to illness or injury can be overwhelming and stressful.  We provide assistance in securing temporary, non-medical services such as personal care, home-delivered meals, caregiver relief, homemaker/light housekeeping, and emergency response devices.   LinkAGE also provides medical transportation for residents of South and Mid Jefferson County.

Dealing with a family member in a long-term care facility can be stressful and challenging.  We provide trained volunteers to protect and promote awareness of the rights of residents.  We can help residents and their family members in securing their rights and ensuring proper health care.

2-1-1 offers assistance in helping you maintain your independence by providing information that guides you through the maze of available services.  Information is available to anyone of any age.
