C A R E   C O O R D I N A T I O N

Services available to seniors, 60 years of age and older who live in the community. A Care Coordinator can assist an older adult to develop a plan of care based on an assessment of care and service needs. Seniors can access the following services on a short-term basis.

  • Non-medical, in-home personal care* - We can provide in home assistance with bathing and dressing.
  • Home delivered meals* - We can make arrangements to have hot, nutritious meals delivered on a temporary or daily basis.
  • Caregiver Relief (Respite Care services)* – We can arrange for someone to assist an older family member to allow the family caregiver time away.
  • Homemaker / Light Housekeeping services* - We can arrange for help with light housekeeping and / or cooking needs on a short-term basis.
  • Emergency Response Devices* – We can provide for the short-term installation of a personal assistance device that allows seniors to summon help in the event of a medical emergency.
  • Medical Transportation (for residents of South and Mid Jefferson County)* – We can provide for rides to doctor appointments, health screenings or other medical visits.
  • Day Activity Program – Seniors who would benefit from supervision can participate in recreational activities, meals and personal care, and monitoring of health status, while providing temporary relief for caregivers.
  • Information on Senior Centers – Locations of Senior Citizen Centers in our region and schedules of social activities are available.

Designing a plan of care is the first step to identifying needs and developing strategies and actions to meet those needs. Care Plans can be developed at any time and should change whenever needs or problems are identified. The steps are listed to the right.

What to do when the problems outlast the solutions?

When the plan of care starts to run out before the need ends, it is usually an indication that your loved one may need long term care. Sometimes the 1st and 2nd steps of the Care Plan won’t change but the 3rd step will need to identify more long-term plans. As soon as you realize that a long-term plan is needed speak with your Care Coordinator. She can help look at options for solutions.

*Services are provided at no charge. However, contributions for services received are encouraged.

STEPS TO PLANNING FOR CARE / SAMPLE CARE PLAN

1. Identify the problems?
"Mom needs help to bathe, fix her meals and clean house."

2. What are the possible solutions?
"Mom will have daily help with her bath daily help with breakfast, lunch and dinner and help to clean the house twice each week."

3. What is the plan?

Baths – Home Health will bathe on Mondays, Wednesdays, and Friday mornings. Daughter will bathe on Tuesdays, Thursdays, Saturdays and Sundays.

Meals - LinkAGE will bring lunch, Mondays through Fridays. Son will bring lunch on Saturdays and Sundays. Daughter will bring dinner daily.

Housekeeping – Son to clean house on Saturdays and Sundays.

Transportation – LinkAGE to provide escort and ride to doctor appointments.
