S E R V I C E S
Welcome to the Caregiver Support Program of
the Area Agency on Aging of Southeast Texas. The Area Agency on Aging provides support and
assistance to family caregivers through the National Family Caregiver Support Program.
Senior citizens and their caregivers have access to services including:
- Non-medical, in-home personal care*
- We
can provide in home assistance with bathing and dressing.
- Home delivered meals*
- We can make
arrangements to have hot, nutritious meals delivered on a temporary or daily basis.
- Caregiver Relief (Respite Care services)*
We can arrange for someone to assist an older family member to allow the family
caregiver time away.
- Homemaker / Light Housekeeping services* -
We
can arrange for help with light housekeeping and / or cooking needs on a short-term
basis.
- Emergency Response Devices*
We
can provide for the short-term installation of a personal assistance device that allows
seniors to summon help in the event of a medical emergency.
- Medical Transportation (for residents of
South and Mid Jefferson County)*
We can provide for rides to doctor
appointments, health screenings or other medical visits.
- Day Activity Program
Seniors who
would benefit from supervision can participate in recreational activities, meals and
personal care, and monitoring of health status, while providing temporary relief for
caregivers.
- Information on Senior Centers
Locations
of Senior Citizen Centers in our region and schedules of social activities are available.
As funding is available, the following
services are made available to family caregivers in support of the assistance that they
provide to their loved ones. All services may be provided based on an assessment and / or
determination of eligibility by the LinkAGE Care Coordinator, in agreement with clients
and caregivers. Individuals interested in discussing the availability of any of these
services should contact the LinkAGE Care Coordinator.
- Caregiver Health Maintenance
(may
include dental care, eyeglasses, hearing aids, and patient care supplies.)
- Limited Medication Assistance
- Limited Residential Repair
(requires
Home Safety Evaluation by Area Agency on Aging representative.)
- Home Safety Evaluation
- Home Safety and Security Modification
Assistance
(requires Home Safety Evaluation by Area Agency on Aging representative.)
- Limited Income Support
(may include
limited emergency assistance with utility expenses.)
- Medical Equipment
(limited to prescribed
equipment not covered by insurance, requires Home Safety Evaluation by Area Agency on
Aging representative.)
- Personal Care Equipment
(may include
hand-held showers, bathroom safety equipment. Moderate home modification promoting
handicap accessibility; requires Home Safety Evaluation by Area Agency on Aging
representative.)
- Limited Yard Work and Home Maintenance
- Home Accessibility Modification
(may include installation of lift chair, wheelchair ramp, modifying appliance controls,
installation of safety rails and grab bars.) * Requires Home Safety Evaluation by Area
Agency on Aging representative.
- Repair or Replacement of Essential
Appliances
(requires Home Safety Evaluation by Area Agency on Aging
representative.)
- Weatherization
(may include repair /
modification or purchase of equipment and supplies that protect client from the effects of
weather and / or conserve energy.)
- Electrical
(may include the
replacement of phone conduits for installation of emergency response, and other limited
electrical home modification.)
